REFERRAL SUMMARY FORM
Please fill out the Patient Referral Form that you can download from the button to the right. This helps summarize the patient’s history and your clinical findings.

BLOOD PANELS
We require pre-anesthetic blood work for all patients. Patients 6 years-old and under will need a CBC and Chemistry panel. Patients over 6 years-old will need a CBC, Chemistry, Thyroid screen and if possible, a Urinalysis.

RADIOLOGY RECOMMENDATIONS
We require Thoracic Radiographs for any cardiac murmur or arrhythmia. If possible a Cardiac ultrasound prior to anesthesia is recommended.

RADIOLOGY REVIEW
Our Veterinarians are available to evaluate Dental Radiographs. Please fill out the Radiology Review form on this website and send via email with attached Radiographs. Please also call us to let us know you sent radiographs since they can get lost in our email.

COMMUNICATION
Referral letters will be sent after any consultation or procedure. We will also forward any blood panel or histopathology report we receive.
